What to expect
Step 01

Book online, or call or text us. If your preferred time isn't listed, reach out — we'll try to fit you in manually.
Step 02
Complete your forms
After booking, you'll receive an email link to a couple of short forms on a HIPAA-compliant, secured platform.
Step 03
Arrive prepared
Fill the forms out at home, or arrive 20 minutes early to complete them at the office.
